Please read the requirements and regulations in the facility use forms completely and return the form to the school that the request is for. You will need to complete the Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19 Form. If you are requesting the use of a classroom(s) or media center, you will also need to complete and sign the Classroom/Media Center Use Agreement. If you are requesting use of a kitchen, you must also complete and sign the Request for Use of Kitchen Facilities form. After filling out and signing the appropriate form(s), please mail, fax or drop them off at the requested school.

To request the use of athletic fields at Central Middle School, please complete this agreement not to park in the cemetery driveway on Vail Road. This agreement must be submitted along with the Use of Facilities Application to the Business Office.

Effective June 25, 2018 until further notice, access to the Parsippany Hills High School Home Side Bleachers will not be permitted.
