According to New Jersey State Mandates, each candidate who wishes to participate in athletics, school athletic squad, or a team MUST be examined by their primary health care provider within 365 days PRIOR to the first practice session. The date of your child's last physical exam can be verified in Genesis Parent Portal by clicking the documents tab and clicking physical exam status notification. Any student athlete with asthma or a food/insect sting allergy that has/or has had an Asthma Action plan or a Food allergy action plan, seizure action plan or any other condition that requires emergency medication MUST also submit the appropriate Medication Packet along with Packet A or Packet B in order to be eligible for play.

A valid baseline concussion testing (IMPACT) is required every two years for High School students. Most 9th and 11th graders will establish a new baseline. More information can be found on the school specific website under Athletics.

All HIGH SCHOOL Fall SPORT athletes must have a completed physical dated 8/12/23 or after for all fall sports. Athletic Registration forms listed below MUST be completed in Genesis, and Packet A or B forms returned to the nurse by 8/1/24 for all fall sport participation.

All MIDDLE SCHOOL FALL athletes must have a completed physical dated after 9/6/23. Athletic Registration forms listed below MUST be completed in Genesis, and Packet A or B forms returned to the nurse by 9/6/24.

2. Preparticipation Physical Evaluation Medical Eligibility Form

3. Preparticipation Physical Evaluation Form (retained by your doctor’s office)
